You certify that you are a licensed healthcare practitioner, or acting on behalf of a licensed healthcare practitioner. You agree not to impersonate any person or entity, misrepresent any affiliation with another person, entity or association, use false headers or otherwise conceal your identity from www.ezmedicaloffice.com for any purpose.
